What is an allergy and how is this different to a food insensitivity?
An allergy is an immune-mediated reaction involving IgE antibodies, which bind to mast cells and trigger immediate release of histamine and other mediators upon exposure to a specific allergen. This causes rapid-onset symptoms like hives, swelling, wheeze, vomiting, or anaphylaxis. Food insensitivities or intolerances are not IgE-mediated and typically involve delayed, non-immune symptoms such as bloating, cramps, or fatigue. While some tests measure IgG antibodies to explore food sensitivities, IgG reflects an adaptive immune response to repeated exposure rather than an immediate allergic reaction. IgE testing does not detect IgG-type food sensitivities or intolerances.
What is an IgE-mediated allergy, and what other types of allergies are not covered?
IgE-mediated allergies are immediate-onset immune reactions involving IgE antibodies and mast cell activation. This test does not detect non-IgE-mediated conditions such as eosinophilic oesophagitis (EoE), food protein-induced enterocolitis syndrome (FPIES), contact dermatitis, or coeliac disease. These conditions involve different immune pathways and often present with delayed GI or skin symptoms.
Can I have both an IgE allergy and a food sensitivity or non-IgE allergy?
Yes, patients can experience both. For example, someone may have an IgE allergy to peanut (causing hives or anaphylaxis) and a non-IgE or sensitivity-related issue with gluten (causing bloating or fatigue). Careful history-taking is essential, as symptom patterns, onset, and systems involved differ between types of immune or non-immune food reactions.
